CODE OF CONDUCT
All participants and guests are expected to abide by Bootz C.E.C. Code of Conduct. Bootz C.E.C. staff reserves the right to temporarily or permanently ban an individual from the Bootz C.E.C. for any conduct which the Bootz C.E.C. staff reasonably believes violates the Code of Conduct or Conduct of these Facility Use Policies.
ASSUMPTION OF RISK
Individuals utilizing Bootz C.E.C. do so at their own risk.
INJURIES/MEDICAL EMERGENCIES
In the event of serious injury, 911 should be called. Bootz C.E.C. staff can provide ice, bandages and a place to rest. In the event that a parent or guardian cannot be reached in an emergency, Bootz C.E.C. reserves the right to seek emergency care for the injured player or guest, including ambulance assistance as may be deemed necessary under circumstances at the time.
PROHIBITED ACTIVITIES & SUBSTANCES
NO outside food or drink, except water or sports drinks in individual plastic containers. NO coolers. No glass bottles. NO sunflower seeds or popcorn. NO alcohol, tobacco products (including e-cigarettes) drugs or weapons. NO smoking in the building or within twenty-five (25) feet of the front of the building. NO harassment or intimidation using words, gestures, body language or menacing behavior.
USER RESPONSIBILITIES
Coaches, and/program providers are responsible for the supervision and control of all participants present at Bootz C.E.C. for the purpose of participating in, supporting or watching their activity. Users are encouraged to report to Bootz C.E.C. staff any items of concern relating to the safety of participants and guests.
CODE OF CONDUCT
We want everyone who enters Bootz C.E.C. to have a safe, rewarding experience. Therefore, we require that everyone adhere to a Code of Conduct that touches upon core values which, if respected, ensure a positive experience for everyone. Failure to adhere to the Code of Conduct will result in a temporary or permanent ban from the Center.
RESPECT
Respect Staff - they are here to provide a safe environment and a rewarding experience.
Respect Others - refrain from any harassing, intimidating, or discourteous behavior toward others.
Respect the Center - treat property and equipment with care, and be clean (pick up after yourself).
Respect yourself - refrain from dress or behavior that may draw attention to yourself or portray you in a bad light.
SPORTSMANSHIP
Exhibit good sportsmanship in all aspects of competition and play, including deferring to officials, abiding by all rules of competition, and refraining from outbursts.
SAFETY
Refrain from any behavior that puts you or anyone else at risk of injury. Wear proper attire, and use proper equipment, for the activity in which you are participating. Report to Center Staff any condition that could create a safety concern.

FOOD AND BEVERAGE POLICY
Bootz C.E.C. is committed to maintaining a clean, safe and healthy environment for its guests and visitors. One of the ways we do this is by strictly enforcing our policies regarding bringing in outside food and beverages in the Center.
OUR POLICY
Outside food and beverages are not permitted in Bootz C.E.C.
EXCEPTIONS
Water and sports drinks in acceptable, sealable, individual containers. We do not allow thermoses, coolers, cups or any other containers that create a risk of spillage.

INSURANCE REQUIREMENTS
All organizations, groups or other persons or entities that rent, lease or otherwise contract to use (“USER”) Bootz Community Event Center (“Bootz C.E.C.”) for any commercial or recreational activity, including but not limited to games, practices, events, tournaments, instruction, camps, performances, markets, bazaars, fairs, exhibitions, displays or other permitted activities, are required to be insured and maintain the kinds and types of insurance that are approved by Bootz C.E.C. and proven to be in force and effect during such time frames as required by Bootz C.E.C..
INSURANCE REQUIREMENTS
USER shall procure and maintain for the duration of the rental period insurance against claims for injuries to persons or damages to property which may arise from or in connection with the use of Bootz C.E.C. facility and the activities of the USER, its guests, invitees, spectators, agents, representatives, employees, or subcontractors.
MINIMUM SCOPE AND LIMIT OF INSURANCE
Coverage shall be commercial general liability type insurance providing coverage on an “occurrence” basis, including property damage, bodily injury and personal & advertising injury. The coverage may be provided by a combination of underlying and umbrella coverage. If the USER maintains broader coverage and/or higher limits than the minimums shown above, the Bootz C.E.C.requires and shall be entitled to the broader coverage and/or higher limits maintained by the USER. Any available insurance proceeds in excess of the specified minimum limits of insurance and coverage shall be available to the Bootz C.E.C. Property damage, bodily injury and personal & advertising injury liability limits shall be in the following minimum amounts:

OTHER INSURANCE PROVISIONS
The insurance policies are to contain, or be endorsed to contain, the following provisions: Additional Insured Status The Bootz C.E.C. , their officers, officials, employees, and volunteers are to be covered as additional insureds on the liability insurance policies with respect to liability arising out of the use of the facility. Primary Coverage For any claims related to this use, the USER’s insurance coverage shall be primary and non-contributory insurance coverage. Any insurance or self-insurance maintained by Bootz C.E.C. their officers, officials, employees, and volunteers shall be excess to the USER’s insurance and shall not contribute with it. Notice of Cancellation Each insurance policy required above shall provide that coverage shall not be canceled, except with notice to the Bootz C.E.C.. Waiver of Subrogation USER is notified that the rental agreement grants to Bootz C.E.C., their officers, officials, employees, and volunteers a waiver of any right to subrogation which any CGL insurer of said USER may acquire against Bootz C.E.C., their officers, officials, employees, and volunteers by virtue of the payment of any loss under such insurance. USER agrees to notify its insurer of this subrogation waiver and, if necessary, obtain any endorsement that may be necessary to enforce this waiver of subrogation, but this provision applies regardless of whether or not the USER has received a waiver of subrogation endorsement from the insurer. Acceptability of Insurers Insurance is to be placed with insurers acceptable to the Bootz C.E.C.. Verification of Coverage USER shall furnish the Bootz C.E.C. with original certificates and amendatory endorsements proving the existence of coverage required by this clause. All certificates and endorsements are to be received and approved by the Bootz C.E.C. at least five days before USER commences activities. Special Events Risks or Circumstances Bootz C.E.C. reserves the right to modify these requirements based on the nature of the risk, prior events, insurance coverage, or other special circumstances.
WORKERS’ COMPENSATION INSURANCE
If USER has employees who will be involved in the use, USER shall provide proof that it has workers compensation insurance meeting Utah’s minimum standards for all employees and contractors engaged by USER for activities at the Bootz C.E.C..
EXHIBITOR'S RULES AND REGULATIONS FOR
CONTRACTED EXHIBIT SPACE
(PLEASE READ CAREFULLY AND SAVE FOR REFERENCE)
Rules and regulations governing the show have been briefly written to advise exhibitors of their rights, restrictions and requirements. Any changes to the rules and regulations must be made in written form prior to the opening of the show.
1. No special signs, partitions, apparatus, shelving, etc. may extend more than 8' above the floor in a booth without the permission of Bootz C.E.C. show management. No similar material should extend above a side rail on a booth over 36", except to the point halfway in the booth to the back wall line. No pins or tacks in drapes, walls, posts, etc. are permitted. Exhibitors must not obstruct the view of an adjoining exhibitor's space, nor permit such exhibits to be placed or operated in any manner offensive or objectionable, in the reasonable opinion of Bootz C.E.C., to the adjacent or surrounding exhibitors or the Show as a whole.
2. No nails or screws may be driven into the floor. (Only double face cloth tape is permitted on the building floor.) NO damage of any nature may be done to the booth structures or to any part of the building. Exhibitors will be held responsible for damages. 3. All aisles must be kept clear of exhibits. Interviews, demonstrations, distributions of literature, etc. must be made inside of the exhibitor’s space. No canvassing, solicitation of business or conference in the interest of business, except exhibiting firms, is allowed. 4. All structural work, such as shelving, standards, display racks, etc. must meet the approval of show management. All materials used must be fireproof and, if necessary, tested by the fire department. All flammable materials such as bunting, trees, drapes, etc., must be fireproofed. Crepe paper is absolutely forbidden, even if fireproofed. All cloth over oneyard square must be fireproofed by order of the fire department. 5. Subletting of space is prohibited. Two or more firms may not exhibit in a single space unless approved by show management.
6. Use of sound equipment, such as microphones, televisions, DVD's, will be permitted where appropriate to the display, provided sound is maintained at not more than "conversation" level: Bootz C.E.C. reserves the right to restrict Exhibitor's use of sound and other devices which may interfere with the best interest of the show as a whole.
7. Exhibitor sales of food and beverages for consumption in the building shall be made only with the permission of the building concessionaire.
8. Many promotional items may be used in your display area - keychains, rulers, etc., - however, no helium balloons, noise makers or food (including popcorn and peanuts) may be given out without permission.
9. The show management reserves the right to refuse space which would in any way detract from the dignity of the show.
10. Exhibitor, at his own expense, must drape back unsightly structures if exposed to view.
11. All labor necessary in setting-up, removing exhibits must be supplied by the exhibitor and must comply with building labor regulations.
12. Exhibitors are to have booths set up and ready by show opening unless prior approval is given by show management.
13. The decision of the show management must be accepted as final in any disagreement between exhibitors or in the decision to remove from the show any exhibitor or his representatives performing any act or practice which, in the opinion of the management, is objectionable.
14. Any exhibitor conducting a FREE DRAWING in the show must prior to the show's opening notify show management about the drawing and prizes to be awarded and after the show's closing notify show management to whom the prizes are being awarded.
15. Exhibitors may not solicit names for drawings where a cash purchase is required to win.
16. All video and still photography of any show exhibit and/or exhibit staff taken by an agent of Bootz C.E.C. will remain the property of Bootz C.E.C.for use in promoting current and future events or for use for any other purpose.
17. All matters not covered in these conditions are subject to the decision of the show management and all exhibitors are to recognize that the show management is to be held harmless for any act of abnormality and for all activities within the actual exhibitor's booth and acts performed by the exhibitor and/or his representative. 18. NO Smoking in the building or twenty (25) ft. from all entrances, Only designated areas.
19. Exhibition Hours. Exhibitors shall maintain a responsible individual or individuals in the Leased Space at all times during designated exhibition hours. Exhibitors shall be responsible for the conduct of any employees, agents, visitors, or guests of Exhibitor on or about the Leased Space. Exhibitors shall cause all such employees, agents, visitors or guests of Exhibitor to be familiar with all Rules and Regulations of the Show. 20. Electrical extension cords must be 3-wire with ground.
21. Proof of Liability Insurance is required for all exhibitors to include: A Certificate of Insurance naming the exhibitor as the certificate holder covering all bodily injury and property damage liability. Bootz C.E.C. will therefore not be reliable for any bodily injury or property damages per participant with the exhibitor.
22. LIABILITY: Bootz C.E.C., nor the Facility in which the exhibits are held can or will be responsible for damage to loss, or theft or property belonging to any exhibitor, his agents, employees, business invitees, visitors or guests. Each exhibitor is expected to carry his own appropriate insurance. Small and valuable exhibit materials should be packed away. Exhibitor agrees to protect, save and hold harmless Bootz C.E.C. their officers, officials, employees, and volunteers, or for whom Bootz C.E.C. contracts to serve as show management and/or the facility forever harmless for any damages or charges imposed for violations of any law or ordinance, whether occasioned by the negligence of the exhibitors or those holding under the exhibitor, except for any damages for charges directly caused by the negligence of any for the foregoing indemnified persons or entities, as well as to strictly comply with the applicable terms and conditions contained in the contract agreement between the facility and Bootz C.E.C.regarding the exhibition premises; and further, exhibitor shall at all times protect, indemnify, save and hold Bootz C.E.C. and/or any losses, costs (including attorney's fees), damage, liability, or expenses arising from or out of or by reason of any accident or bodily injury or other occurrence to any person or persons, including the exhibitor, its agents, employees, and business invitees, which arises from or out of or by reason of said exhibitor's occupancy and use of the exhibition premises or a part thereof, except for those matters directly caused by the negligence of the foregoing indemnified person or entities.
